Please have the following information ready for the scheduling team:

• Patient’s demographic Information
  
• Insurance information
  
• CPT codes for the procedure to be performed.

Should you choose to fax the information and your requested time is not available, your office will be notified within 24 hours.

Same day surgeries can be performed at Weston Outpatient Surgery Center as long as it is scheduled by 4:00pm. This is to allow adequate time for insurance authorizations and additional staffing.

Early morning add-ons can also be accommodated. Simply provide us with the needed information by 4:00pm the previous evening and we will handle the rest!
